악골에 발생한 백악질 및 골화성섬유종의 임상 및 방사선학적 고찰

Abstract

저자들은 1980년 1월부터 1995년 12월까지 연세대학교 치과대학병원을 내원하여 임상적,
방사선학적 그리고 조직병리학적으로 백악질 및 골화성섬유종으로 진단받은 13예를 대상으
로 임상적, 방사선학적 그리고 조직병리학적 소견을 분석한 결과 다음과 같은 결론을 얻었
다.
1. 발생연령은 29세 에서 65세까지로 다양하게 나타났으며 평균연령은 44세이고, 연령별로
는 30대에서 6예로 가장 많이 나타났다. 그리고 성별분포는 여성에서 11예, 남성에서 2예
5:1의 비율로 여성에서 호발하였다.
2.내원당시의 환자의 주소로는 안면부의 종창만을 주소로 내원한 경우가 6예로 가장 많았
고 무증상으로 우연히 발견한 경우도 3예에서 나타났다.
3. 방사선학적 소견으로는 하악에서 12예로 상악의 1예에 비해 우세하게 나타났으며 발생
부위별로는 소·대구치부에서 11예로 가장 많이 발생하였고, 병소의 경계는 분명한 경우가
8예, 덜 분명한 경우가 5예였으며 경계가 불분명한 경우는 없었다. 성숙정도에 따른 분류에
서는 방사선투과성 병소가 1예, 흔합성 병소가 9예, 방사선불투과성 병소가 3예에서 나타났
다.
4. 조직병리학적 소견으로는 골화성섬유종이 7예, 백악질골화성섬유종이 4예, 그리고 백
악질 화성섬유종은 2예로 골화성섬유종으로 진단되어진 증례가 가장 많았다.

The purpose of this study was to know the proper diagnosis and to establish the
treatment plan of cementifying and ossifying fibroma in the jaws through the clinical,
raiological, and histopathologic considerations. The authors compared and analyzed the
clinicoradiologic features of the thirteen cases of cementifying and ossifying fibroma,
diagnosed at the Dental college hospital in Yonsei university, Seoul, Korea, during the
period from 1980 to 1995.
The obtained results were as follows
1. Cementifying and ossifying fiblroma occured in the mean age, 44 years, ranged
from 29 to 65 years and the male to female ratio was approximately 1:5.
2. Swelling was the most common frequent presenting complaints. Other reported
symptoms included pain, tooth mobility and asymptom.
3. The frequency of the lesions was twelve cases in the mandible and one case in the
maxilla. And eleven of thirteen cases were distributed on the premolar and molar region.
4. Radiologically, eight of thirteen cases were well defined lesions, five cases were
relatively well defined lesions. And nine of thirteen cases were mixed lesions, three
cases were radiopaque lesions, and only one case was purely radiolucency lesion.
5. Histologically, semen of thirteen cases were classified ossifying fibroma, four cases
were cemento-ossifying fibroma, and two cases were cementifying fibroma.
